<h2 style="font-weight: 400;" align="justify">Valve-within-valve — How does it work?</h2>
<p style="text-align: justify;">Similarly to native valve TAVR, the valve within valve process utilises the existing aortic bioprosthesis as a docking station for the second valve. The second valve is loaded into the delivery system and deployment technique is dependent on the type of bioprosthesis the original valve was. The second valve will either be deployed inside the existing valve using a balloon-expandable valve system or a self-expandable valve system. The advantages of these valve within valve procedures are that they are far less invasive compared to a redo AVR. Generally, there are reduced morbidity and mortality rates and reduced hospital stay. These patients are often very elderly and have multiple co-morbidities, so a less invasive procedure is far more suitable for them. A valve within valve procedure can also be more appropriate for a bioprosthesis in the mitral or tricuspid position. Once the second valve has been deployed, the hemodynamics are assessed to ensure a good functioning valve with no gradients below 30 mmHg or aortic regurgitation.</p>
<p style="text-align: justify;">An elderly patient with severe aortic stenosis post bioprosthesis insertion who now presents with congestive cardiac failure. Echo shows a significant aortic valve gradient and a low ejection fraction. This patient would most likely further decompensate with a more invasive procedure such as a redo AVR, so a valve within valve TAVR procedure would be more appropriate for them.</p>
<h2 style="font-weight: 400;" align="justify">How is TAVR or TAVI different from standard valve replacement?</h2>
<p style="text-align: justify;">In terms of the procedure itself, TAVR/TAVI is distinct from standard valve replacement surgery. The standard procedure requires the surgeon to open the chest wall for direct access to the heart, and the patient is placed on a heart-lung bypass machine in order to oxygenate the blood and pump it around the body while the surgeon stops the heart. With TAVR/TAVI, the procedure of implanting the valve is done while the heart is still beating. The<a href="https://www.tunisiamedicaltravel.com/tavi/"> TAVI</a> approach can be performed in several anesthetic settings: local anesthesia with a sedative, general anesthesia, or occasionally with just heavy sedation. This limits the impact of the surgery itself on the patient and generally makes it easier for those in a frail state to undergo the procedure. Due to the minimally invasive nature of TAVI, it is particularly beneficial to those at very high risk of surgery, providing palliative treatment to patients who also have cancers or other advanced systemic diseases. This is in sharp contrast to the surgery, during which the patient must be in relatively good health to withstand the procedure and the following recovery.</p>
<h2 style="font-weight: 400;" align="justify">What is involved in a TAVR procedure?</h2>
<p style="text-align: justify;">A TAVR procedure requires only a small incision and is relatively less invasive than open-heart surgery. Doctors can perform TAVR through multiple approaches: transfemoral (through the femoral artery), transapical (through the aorta, usually in patients who have a problem with the femoral artery), or another alternative method. The TAVR procedure can be done under general anesthesia, so the patient is asleep during the procedure. TAVR may also be performed with local anesthesia and/or mild sedation. The catheter-based procedure follows many of the same basic principles as a standard cardiac catheterization. For example, the patient will be closely monitored using advanced imaging technology. A team of interventional cardiologists and cardiac surgeons will work together to ensure the best possible treatment. The length of the procedure can vary, but it generally takes a few hours. Following the procedure, the patient will be monitored in an intensive care unit or a step-down unit in the hospital. Patients who have undergone TAVR will need to undergo antimicrobial treatment to prevent infection of the heart lining. This medication will be administered intravenously and may continue at home.</p>

<h2 style="font-weight: 400;" align="justify">Who&rsquo;s a good candidate for this type of valve surgery?</h2>
<p style="text-align: justify;">People who are good candidates for TAVR include those who have been diagnosed with severe aortic stenosis and are symptomatic from their aortic valve disease. It is a condition that can affect people to varying degrees, but once symptoms such as chest pain, or pressure, shortness of breath, fatigue, lightheadedness, or passing out occur, it is a serious and life-threatening situation. Without aortic valve replacement and currently TAVR, 50% of patients will not survive more than an average of 2 years after the onset of symptoms, 20-30% will survive 5 years, and only about 20% will be alive at 8 years from the onset of symptoms. Other patients who are not good candidates for SAVR are also good candidates for TAVR. This can include patients of any age with lung disease, porcelain aorta, chest radiation, prior heart surgery, and patients who have other medical conditions or a low ejection fraction which would make open heart surgery very high risk. Due to its less invasive nature, TAVR is often better for high-risk patients in general. At this time, TAVR is only FDA approved for high-risk patients. The heart team will take into account the number and extent of these risk factors for each individual patient. This will be weighed against benefits such as increased life expectancy and quality of life, and ability to do more day-to-day activities. An informed consent process between the physician and the patient will take place in order to reach a shared decision about whether or not TAVR is the best choice.</p>
<h2 style="font-weight: 400;" align="justify">Is there financial assistance available for people in need of TAVR or TAVI valve replacement?</h2>
<p style="text-align: justify;">TAVI or TAVR valve replacement is a relatively new procedure that can be quite expensive. It&rsquo;s important to be aware of your options for financial assistance before the procedure. The cost of TAVR or TAVI valve replacement can vary quite a bit, but it is generally between $30,000 and $50,000 in the United States. This cost may or may not be covered by insurance. For those with Medicare Part B, the TAVR procedure is now covered, although there may be co-pays involved. For those with private insurance, it&rsquo;s a good idea to inquire with your provider about whether the procedure is covered. In some cases, the TAVR procedure may be covered by insurance, but only at specific hospitals. Before making any decisions about TAVR or TAVI valve replacement, be sure to discuss coverage options with your doctor and insurance provider. <h2 style="font-weight: 400;" align="justify">What is angiography and how is it done?</h2>
<p style="text-align: justify;">An angiogram of the heart is a special X-ray test. It is done to take pictures of the blood flow in an artery or a vein. This test is carried out using a technique called catheterization which involves the use of catheters and X-ray. An angiogram is done to find out if there is any abnormality in the blood vessels, to detect a blockage, to know the extent of a blockage, and to decide the most suitable treatment for the patient. An angiogram may be done in any blood vessel throughout the body. Special types of angiograms may be studies of blood vessels in the brain, eye, lung, kidneys, or other parts of the body. However, this discussion is focused on coronary angiogram, the blood vessel around the heart. A coronary angiogram is usually carried out in a cardiac catheterization lab. This is usually carried out at the same time as angiography.</p>
<h2 style="font-weight: 400;" align="justify">What happens during coronary angiography?</h2>
<p style="text-align: justify;">The doctor will make a small puncture in your skin and insert a catheter (a long, thin, plastic tube) into a blood vessel. The doctor will carefully guide the catheter up into your heart. You may feel some pressure in your groin area when the doctor is advancing the catheter. This is normal. Let your doctor know if you feel any pain in your chest, arm, jaw, or throat. These sensations may be a sign that the catheter is touching a blood vessel. Upon arrival in the catheterization lab, you will change into a hospital gown and remove any jewelry. You will be asked to lie on a procedure table. A nurse will connect you to several monitors, which will be used to check your vital signs (heartbeat, blood pressure) during the procedure. Local anesthesia will be used to numb the area in your groin where the doctor will be working. You will be awake during the procedure. However, the amount of sedative given will vary, depending on whether you are having an angiogram because of a recent heart attack or unstable angina. During <a href="https://www.tunisiamedicaltravel.com/coronary-angiography/">coronary angiography</a>, you&rsquo;ll be closely monitored and cared for. This test is usually done in a catheterization lab in a hospital. You will be asked not to eat or drink anything for 6 to 8 hours before the procedure. In most cases, you will need to stay in the hospital overnight. But if your angiography is scheduled late in the day, your doctor may ask you to stay overnight before the procedure.</p>
<h2 style="font-weight: 400;" align="justify">How long will an angiography take?</h2>
<p style="text-align: justify;">After the procedure, patients are moved to a recovery room and are observed. They can often remove the sheath in common femoral artery themselves at the end of the observation period, and then the puncture site is uncovered for another 2-5 hours. Observation time is highly variable depending on the nature of the angiography and the clinical condition of the patient. Ambulatory patients without adverse symptoms may leave the hospital after an hour of observation. Inpatients may return to their hospital room after 4-6 hours of observation. If the patient was admitted purely for the angiography and it was uncomplicated, they are generally discharged later that day. As with any procedure, there are risks of both inpatient and outpatient angiography, that could lead to a significant change in the duration of hospital stay. If a significant procedure such as angioplasty, stenting, or thrombolysis is performed as a result of the angiography, the length of stay can be 1-2 days longer than procedural counterparts.</p>

<h2 style="font-weight: 400;" align="justify">When can I go home after angiography?</h2>
<p style="text-align: justify;">Depending on the type of procedure done, some people can go home the same day as their angiogram, while others may need to stay in the hospital overnight. Those who have an angioplasty and stent usually need to stay in the hospital for 1 to 2 days. Although this is done through a puncture in an artery in the arm or leg, it is still important to remain still for several hours after the procedure to decrease the risk of bleeding at the catheter site. Activities after going home can usually be done as the person is able to do. Restrictions are typically related to not lifting heavy objects or things greater than 10-15 pounds and not lifting the arm above the shoulder if the puncture was in the arm. Medication is not usually prescribed after the procedure to have an effect on the ability to care for oneself. Most people who have angiography have a rapid recovery and are able to return to their usual activities within a few days. Follow-up care after angiography and potential interventions is based on the clinical findings that were unveiled. This would be a topic to discuss at the time of the procedure or at the time of the follow-up visit.</p>
<h2 style="font-weight: 400;" align="justify">Risks involved with angiography and angioplasty</h2>
<p style="text-align: justify;">Iodine contrast medium can cause an allergic reaction in approximately 1 in 40 patients. While most of these reactions are mild, it is important to be vigilant as severe reactions can occur and very rarely, death. Allergy to contrast medium is not an issue with angiography alone. Venous thrombosis, contrast medium inadvertently injected outside the coronary arteries, and radiation exposure to the testes with potential effects upon future fertility are other rare complications of angiography. High quality referrals to the cardiologist can minimize the chance of these adverse events by ensuring that the test is performed in an appropriate patient and that the most experienced operator available carries it out. Simulation with a non-invasive test before angiography is another way to reduce the risk of complications, though this must be balanced against the small risk of the non-invasive test itself. Angiography and angioplasty are known as relatively safe procedures. Major complications combine a death rate of about 1 in 500. Stroke is caused in about 1 in 100 people. Heart attack is very rare, while developing a need for emergency bypass surgery is about 1 in 300. Contrast-induced nephropathy is an important cause of morbidity and mortality after coronary angiography. Premenopausal women, patients with pre-existing renal impairment, and people with diabetes are particularly at risk. Measures to reduce the risk of contrast-induced nephropathy include staying well-hydrated and limiting the amount of contrast used.</p>

<h2 style="font-weight: 400;" align="justify">What is angioplasty?</h2>
<p style="text-align: justify;">In modern medicine, angioplasty is recognized as a well-recognized used process to improve heart situation and improve towards a better life. Generally, angioplasty is executed where coronary arteries (which supply oxygenated blood to heart muscle) are blocked due to deposition of fat. Because overtime these gets clogged and the blood flow gets reduced taking the situation it to more severe form called coronary artery disease (CAD) which can further lead to heart attack. Here is where angioplasty is an immediate alternate and lesser risky measure than bypass surgery (certain individuals even opt for it), it&rsquo;s a treatment to help to restore blood flow to your heart. Reducing damage to the heart muscle and treating CAD will also decrease the risk of heart failure in the future. Angioplasty can be performed at the same time as a PTCA(Percutaneous transluminal coronary angioplasty)./p&gt;</p>
<h2 style="font-weight: 400;" align="justify">Types</h2>
<p style="text-align: justify;">There are two primary types of angioplasty procedures. The most common is balloon angioplasty. In this procedure, a catheter is threaded through the arteries to the site of the blockage. The catheter has a tiny balloon at the end that is inflated once the catheter is in the correct position. Inflation of the balloon is done on site using a guide wire. This flattens the plaque against the wall of the artery, widening the artery lumen and restoring blood flow. After the balloon is inflated, it is deflated and removed from the body. Sometimes angioplasty using a drug-eluting balloon will be used in which medication is released through the balloon in order to better affect the artery. The second type of angioplasty is called stenting. In this procedure, once the balloon has expanded the artery, a wire mesh stent is also expanded and left at the site of the blockage to keep the artery open. Some stents are drug-eluting. This means that they release medication to help keep the artery from narrowing again. Balloon angioplasty does not involve leaving foreign objects within the body and is therefore less invasive than stenting, though stenting has a higher success rate in reducing ischemic events.</p>
<h2 style="font-weight: 400;" align="justify">How to prepare?</h2>
<p style="text-align: justify;">If there is any possibility that you are pregnant or you have allergies to x-ray contrast medium, iodine, or seafood, please inform the nurses and your doctor. On the day of your angiogram. You will then be taken to the Angiography Suite where you will change into a hospital gown and have an IV line inserted. Your doctor will see you before the procedure to explain what will happen and for you to ask any further questions. After the angiogram is complete and it has been determined that you require an angioplasty, your doctor will admit you to the hospital and arrange another date for the procedure to be performed. At this point, your doctor will ask for your informed consent to perform the angioplasty. If you take warfarin or anti-inflammatory drugs such as aspirin or ibuprofen, it is very important that you inform your doctor and the nurse. It may be necessary to stop taking them and start on other medication prior to the procedure. You will be asked not to eat for six hours before the procedure. You can drink water and take your tablets as normal. Bring all your medications with you on the day of the procedure.</p>
<h2 style="font-weight: 400;" align="justify">Procedure</h2>
<p style="text-align: justify;">Access to the blood vessels is gained through a very small incision in the skin, usually at the groin. A plastic tube called a sheath is inserted into a blood vessel, and the flexible catheter is advanced through the sheath into the blood vessels. In some cases, access to the blood vessel is through a needle puncture of an arteriovenous graft or fistula, or through a puncture in a blood vessel in the arm or leg. This approach is used when angioplasty is to be performed on a graft blood vessel. Angioplasty is performed in a specially equipped medical suite or a catheterization laboratory. The patient is usually awake during the procedure, and the degree of sedation is variable depending on the preference of the operator and the patient. Blood pressure and heart rate are monitored during the procedure. The patient is positioned on the procedure table, and the skin is cleansed with an antiseptic solution. A sterile drape is placed around the area. A local anesthetic is injected into the skin.</p>

<h2 style="font-weight: 400;" align="justify">Risks</h2>
<p style="text-align: justify;">The <a href="https://www.tunisiamedicaltravel.com/angioplasty/">angioplasty procedure</a> results in a low rate of major complications, but it is important for the patient to be informed about them. These risks include death for 1 person in a 1,000. This risk is related to the degree of narrowing of the coronary arteries. Heart attacks occur in about 1-2% of patients undergoing angioplasty. The risk of heart attack is higher when angioplasty does not successfully open an occluded artery, when a large amount of the heart muscle is at risk because of the status of the narrowed artery, and in patients with unstable angina. Damage to an artery. During angioplasty a small amount of damage occurs in the artery (about 15% of lesions undergoing angioplasty are associated with some damage to the artery). This will cause either a small clot to form at the site of injury hence causing a heart attack, or occlusion of the artery at a later date from scarring and constriction of the artery at the site of injury. High lesion complexity is associated with an increased risk of prolonged ischemia and emergency bypass surgery during angioplasty. Other important risks to note are those for certain subsets of patients. Patients who are diabetics have a threefold increase in mortality from coronary events occurring during or after angioplasty. This is because atherosclerosis in diabetics is more diffuse and leads to a higher possibility of occlusion of the arterial lumen from the debris becoming dislodged during angioplasty. Severely symptomatic patients (ie. patients with class IV angina) have a higher risk of myocardial infarction during angioplasty and more serious complications in the course of those MI&rsquo;s.</p>
<h2 style="font-weight: 400;" align="justify">Recovery</h2>
<p style="text-align: justify;">It is common to have a sore and slightly numb groin for a day or two. You may also develop a benign hematoma (collection of blood) that causes bruising at the site. Apply a cold pack (crushed ice in plastic bag) to the area for 15 minutes each hour and take it easy for the first couple of days after the procedure. You can resume all your usual activities including bath or shower on the third day after having seemed improvement in the soreness at the biopsy site. Avoid heavy lifting or vigorous exercise for a week after the procedure. This will reduce the chance of re-bleeding into the artery. On the seventh day after the procedure, you can usually restart aspirin or other anti-platelet medication if it was stopped for the angioplasty.  When you first wake up, the nurse will probably remove the introducer sheath from the artery in your groin. To do this, pressure is applied to the biopsy site for 20 to 30 minutes. The nurse then puts a clamp on the artery, and you need to keep your leg straight for another four to six hours to avoid bleeding from the biopsy site. This is the only time you have restricted activity. You must remain lying in bed for the first hour and a half after the clamp is removed.
